For both the 87-and 91-product underground storage tanks, beneath Recordkeeping, change"no" to <br /> "Yes"for Visual Inspection Records since this facility is keeping daily inspection records. <br /> This is a minor violation. <br /> 109 CCR 2637 Failed to test secondary containment. <br /> Secondary containment testing was last performed on August 28, 2015 and the secondary containment of under <br /> dispenser containment 3/4 failed the thirty minute test. Repair and retesting has not been completed and is twelve <br /> months past due. Secondary containment testing is required once every 36 months. Immediately contact a properly <br /> licensed, trained, and certified contractor to repair/ replace 3/4 under dispenser containment under permit and <br /> inspection of the Environmental Health Department(EHD). <br /> This is a Class II violation. <br /> 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on August 12, 2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by September 11, 2015. <br /> This information has not been received resulting in a non-compliant status for this facility. An operator that receives <br /> an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective <br /> actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting <br /> corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. Outstanding <br /> violations from the 2015 monitoring system certification include: The Environmental Health Department did not receive <br /> the 2014 Return to Compliance certification for outstanding violations; and an approved copy of the monitoring plans <br /> were not available for inspection. The second violation will be removed since a monitoring plan was available today <br /> during the inspection. <br /> This is a repeat violation, Class II. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator listed the date of 9/22/15 for"Spill container structure (bucket) testing was completed within <br /> the past year"on the October 26, 2015 through July 27, 2016 designated operator monthly inspection reports. The <br /> date that the 91-product spill container(that initially failed during the annual testing date of 8/12/15) passed was <br /> 8/31/2015. The designated operator listed "NA"for"Spill container structure (bucket)testing was completed within <br /> the past year" on the August 28, 2015 and September 28, 2015 designated operator monthly inspection reports. <br /> During the monthly inspection, the designated operator shall check that all required testing and maintenance for the <br /> UST system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> Page 4 of 5 <br />
