The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Contact Garrett Backus to arrange the following inspection(s) (a 48 hour notice is <br />required): Inspection of the functionality of the 323 sensor. <br />3. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />4. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br />date is required, a written request must be submitted to this office prior to the expiration <br />date. <br />5. All components must be certified by an independent testing organization and labeled as <br />required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of <br />Regulations (CCR), Title 23 Chapter 16 Table B for complete list). The component must <br />bear the appropriate mark as required by the independent testing organization. <br />6. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />7. A "Certification of Installation / Modification" shall be submitted by the owner or the <br />owners' agent [pursuant to (CCR) 2635(d)] before this repair will be considered complete <br />by the EHD. <br />Page 1 of 2 <br />
