LEAK RESPONSE PLAN <br />Underground Storage Tank Monitoring Program <br />FACILITY NAME: %I lc Aj o M.&n, /4 e -:7 <br />FACILITY ADDRESS: z. d ck d <br />The following procedures shall be followed by all personnel in the event of a leak or suspected <br />leak: <br />1. Store personnel shall notify the Franchisee immediately if a leak is suspected. <br />2. Franchisee will determine if the suspected leak is evidenced by inventory records or <br />detected by monitoring equipment. <br />A. If inventory problems are encountered, Franchisee will notify Quik Stop District <br />Advisor to review records. No further action is necessary if the problem is due to <br />arithmetic error. <br />B. If equipment indicates a possible leak, Franchisee will contact Quik Stop Gasoline <br />Department for maintenance dispatch to the site. No further action is necessary if <br />the problem is due to malfunctioning equipment. <br />3. Continued indications of a suspected leak shall be reported to the Quik Stop Director of <br />Environmental Affairs for review of site conditions and initiation of additional <br />investigative or corrective action. <br />4. All subsequent actions shall be under the authority and supervision of the Director of <br />Environmental Affairs. The Director shall determine the appropriate steps to be taken <br />relative to specific site conditions. The steps taken by the Director may include some or <br />all, but not limited to, the following actions: <br />A. Tank and/or line integrity testing. <br />B. Removal of product from the tank and lines. <br />C. Local agency notification and submittal of an Unauthorized Release Report. <br />D. Initiation of emergency abatement procedures. <br />E. Repair, replacement, or permanent abandonment of the UST system. <br />F. Initiation of site assessment or remedial activities, submittal of work plans. <br />Responsible on-site party, Franchisee: AAre j ARdied <br />
