Contact the local fire department for their permit requirements. <br /> 2. Samples must be taken and analyzed as stated on pages 4 and 5 of the permit application. <br /> 3. Per permit application,Horizon Environmental Inc will perform the soil sampling and Kiff Analytical will <br /> perform the analytical tests. <br /> 4. All analytical results must come from a California certified laboratory or this office will not accept <br /> them. <br /> 5. Environmental Health Department(EHD)must witness the soil sampling. Contact EHD to arrange this <br /> inspection appointment. <br /> Installation Conditions: <br /> 1. Contact the local fire department and San Joaquin Valley Air Pollution Control District for applicable permits. <br /> 2. Any changes in information or scope of work must be submitted to EHD prior to commencing work. <br /> 3. A site health and safety plan must be at the work location during construction. <br /> 4. Primary and secondary piping must pass a 30-minute pressure test witnessed by EHD <br /> 5. EHD must witness a 24-hour lake test on the new dispenser pans. <br /> 6. All piping and connections shall bear a UL label. <br /> 7. All metal equipment in contact with backfill must have corrosion protection. <br /> 8. A third party certified piping line tightness test must be performed and results submitted prior to scheduling <br /> the final inspection <br /> 9. If automatic pump shut down is provided it must satisfy 2636(g) of California Code of Regulations(CCR). <br /> 10. A final inspection must be performed prior to returning this station to operation. <br /> 11. Contact Dina Abate EHS at(209)468-3468 to arrange all inspections. Allow 48-hour notice for all <br /> inspections. <br /> 12. Permit expires May 30, 2003.
