Fremont, Stockton <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS II, or MINOR -Notice to Comply) <br />This is a Routine and Monitoring Certification Inspection. The following are violations observed during <br />this inspection: 5,7,21, 23b,23c,23d <br />5. The monitoring and response plans on site are not current. The monitoring and response plans must <br />be current and approved by the EHD. Complete and submit a copy of the monitoring and response <br />plans for approval by the EHD. Corrected on site. <br />7. Log for the L-2 alarm on July 5, 2011 was not found on site. These records shall be kept on site for at <br />least 3 years. Immediately locate and maintain the on site and submit copies to the EHD by September <br />20, 2011. <br />21. (Repeat) Approximately 11 gallons of liquid was observed in the 87 -octane fill sump, and about 1 <br />cup of liquid was observed in the 87 -octane STP sump. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br />and manage it accordingly. Ensure that the sumps are maintained free of liquid. Corrected on site. <br />23b. The May, June, and August of 2011 monthly designated operator reports indicated Sciume Darren <br />who performed the inspections did not have a current ICC certification at the time of the inspections. A <br />designated operator shall have current ICC certification at the time of the monthly inspections. Ensure <br />that any designated operator who performs monthly inspections at this facility has current ICC <br />certification, and submit documentation to the EHD by showing that he had current ICC certification <br />when the monthly inspections were performed. Submit a copy of Sciume Darren's ICC certification <br />September 20, 2011. <br />23c. The designated operator failed to document the July 5, 2010 L-2 fuel alarm and check that it was <br />responded to appropriately on the designated operator monthly inspection report. During the monthly <br />inspection, the designated operator shall review the alarm history for the previous month, check that <br />each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators <br />performing monthly inspections at this facility are including all of the required information on the reports. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspe r: <br />Received By: <br />MA " ) <br />Title: <br />EHD 234003 li(ev48//1Ol / / CONTINUATION FORM <br />
