The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent). Proper drainage must also be verified. <br />Please be aware that the facility cannot receive fuel in the 87 octane tank prior to <br />the EHD inspection and approval. <br />4. If concrete will be broken near the UST, a secondary containment test must be <br />performed on the UST once concrete is poured in accordance with the manufacturer's <br />guidelines. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />
