Fremont, Stockton <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />This is a report of the annual Routine UST inspection at the referenced facility. Following is a Class II <br />Repeat violation: <br />6. Approved copies of the monitoring and response plans were not found on site. The approved copies <br />had been hand delivered to facility by San Joaquin County Environmental Health Department (EHD) on <br />September 2, 2009. <br />- Copies of these plans shall be retained on site at all times. <br />* Immediately locate and retain a copy on site. <br />NOTE (Following are not violations) <br />The facility's contractor, Walton, was unable to provide a technician to conduct the Monitoring <br />Certification (MC) today. The MC had been previously scheduled with EHD, but EHD was not informed <br />of the cancellation until 30 minutes after arrival at the facility. EHD informed facility attendant that the <br />annual Routine UST inspection will be conducted today, and the time for the MC, when rescheduled, will <br />be billable at current EHD hourly rate. Please be aware that the standard 48 business hour notification <br />for MC schedule still applies. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by August 30, 2010. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATE WILL BE BILLED AT THE CURRENT HOURLY RATE ($115). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TI AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspe <br />Received By: <br />Title: 15 <br />v <br />EHD 23-02-003 Rev /01/10 CONTINUATION FORM <br />
