The permit has been approved with the following <br />conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Jeff Wong at (209) 468-0335 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of all sensors associated with the proposed wiring <br />repair and above-mentioned alarms must be verified to ensure that monitoring has <br />been restored to meet the requirements of Title 23 California Code of Regulations <br />and the Health & Safety Code. <br />Please be aware that the facility cannot operate the UST prior to the EHD <br />inspection and approval. <br />4. If concrete will be broken near the UST or piping, a secondary containment test must be <br />performed on the UST or piping once concrete is poured in accordance with the <br />manufacturer's guidelines. <br />5. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
