After speaking with the operator, he stated that the site has issues with the 91 product line and that are <br /> working on making the repairs. He also stated that they are no longer using the tank nor the lines because of an <br /> alarm on the 1-5-91 STP sump that was enable three days after they started using the tank. He stated that they <br /> stopped using the tanks as soon as the sensor went into alarm. The underground storage tank system shall be <br /> operated to prevent unauthorized releases. Immediately contact a certified contractor to make the needed repairs <br /> to the 91 product line and to remove the fuel that is inside the 91 product STP sump. <br /> This is a Class II violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo box float and chain in the 87 product in UDC 3/4 and 7/8, 89 product in UDC 7/8 sump failed to stop the <br /> flow of product at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible <br /> and visual alarm or stop the flow of product at the dispenser when it detects a leak. The service technician adjusted <br /> the chain in UDC 1/2 for the 89 product, cleaned the float for UDC 7/8, 87 product and UDC 3/4 product and verified <br /> functionality. <br /> This is a repeat violation, Class II. <br /> 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. <br /> approximately 30 gallons of fuel was found on the 91 product STP sump, indicating a leak in the primary piping. All <br /> primary containment for the UST system must be tight. Immediately have a properly licensed,trained, and certified <br /> contractor repair or replace the failed component under permit and inspection of the EHD. <br /> This is a Class II violation. <br /> 319 CCR 2635(b) No spill bucket, failed to contain five gallons, or failed to provide a means to keep it empty. <br /> The 91 product spill container is not equipped with a properly functioning drain valve. All USTs shall be equipped <br /> with a spill container that has a minimum capacity of five gallons and a drain valve that allows drainage of the <br /> collected spill into the primary container or provide a means to keep the container empty. Immediately contact a <br /> properly licensed, trained, and certified contractor to install an approved spill container under permit and inspection <br /> of the EHD. If a spill container can't be installed immediately,there is a possibility that the 91 product UST system <br /> may be red tagged to prevent fuel inputs. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 29th, 2018. <br /> Document reviewed: Monitoring system certification test results 2015, 2016 and 2017, secondary containment <br /> test results from 2017, employee training, response plan, monitoring plan and designated operator monthly <br /> inspection reports from June 2017, July 2017-June 2018. <br /> FA0003730 PR0231125 SCO01 06/29/2018 <br /> EHD 23-01 Rev.10/31/2017 Page 7 of 8 Underground Storage Tank Program OIR <br /> 1868 E.
