The permit has <br />been approved with the following conditions. <br />Conditions of approval: <br />1. Contact the local fire department, local planning or building department and the San <br />Joaquin Valley Air Pollution Control District for any applicable permits. <br />2. Any changes shall be submitted to the San Joaquin County Environmental Health <br />Department (EHD) for approval prior to commencing work. All new addenda supercede <br />the previous ones. <br />3. Send an updated scope of work to the EHD, after excavation of the piping, with the parts <br />needed for the repair. Include cut sheets and UL listings for each part. <br />4. Contact Garrett Backus (468-2986) to arrange the following inspection(s) (a 48-hour <br />notice is required): <br />a. A soap test to verify the pipeline integrity. Isolate and pressurize the pipe <br />following the manufacturer's guidelines. <br />5. If concrete is being broken near the UST, pipeline and tank integrity testing must be <br />performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br />guidelines. Results must be received by the EHD within seven days of returning the <br />facility to operating condition. <br />6. All components must be certified by an independent testing organization and labeled as <br />required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of <br />Regulations (CCR), Title 23 Chapter 16 Table B for complete list). The component must <br />bear the appropriate mark as required by the independent testing organization. <br />7. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />Page 1 of 2 <br />
