RENS <br />Linda Turkatte, RENS <br />SR0063189 <br />The San Joaquin County Environmental -Health Department (EHD) has -completed the review -of <br />the AMENDED application for a permit to replace the 91 spill container, the 91 tank drop tube <br />and overfill prevention flapper valve on the underground storage tank (UST) system at the <br />above noted address. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Overfill prevention verification: Measurement of 91 product drop tube to verify 95% <br />tank volume shut off. <br />b. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br />backfilling. <br />c. Spill bucket test after backfill: Verify minimum 5 -gallon capacity and pass a one hour <br />hydrostatic test (or equivalent). Proper drainage must also be verified. <br />d. Since concrete will be broken over the 91 product tank, a secondary containment <br />test must be performed on the 91 product tank once concrete is poured in <br />accordance with the manufacturer's guidelines. <br />Please be aware that the facility cannot receive fuel in the 91 tank prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />
