Financial responsibility documents have not been submitted to the San Joaquin County <br />Environmental Health Department (EHD). Current financial responsibility documents are required to be <br />submitted annually. Complete and submit a copy of the financial responsibility by August 15, 2011. <br />7. Secondary containment testing results for work done by Walton Engineering on 3-3-11 and 5-18-11 <br />were not found on site. These records shall be kept on site for at least 3 years. Immediately locate and <br />maintain these secondary containment test results on site and submit copies to the EHD by August 15, <br />2011. <br />17. The 89 and 91 grade line leak detectors failed to detect a leak when tested. All line leak detectors <br />shall be capable of detecting a 3 -gallon per hour leak at 10 psi. The leak detectors were replaced and <br />retested during the inspection. Obtain a permit from the EHD for this work within one business day. If a <br />permit is not obtained within one business day, a "Failure to Obtain a Permit" penalty will apply. <br />23c. The designated operator failed to document all the alarms on the April 2011 designated operator <br />monthly inspection report and failed to check that they were responded to appropriately. The missing <br />alarms include: L1 - 91 STP sump - fuel alarm - March 24, 2011. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was <br />documented and responded to appropriately, and attach a copy of the alarm history with documentation <br />taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />Note <br />1.
