SAN JOAQUIN COUNTY 4 <br />ENVIRONMENTAL HEALTH DEPARTMENT <br />600 East Main Street, Stockton, California 95202-3029 <br />Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sigov.org/ehd/unitiii.htmi <br />CONTINUATION FORM <br />OFFICIAL INSPECTION REPORT <br />Page: 3 of 3 <br />Date: 03/03/11 <br />Facility Address: 1469 E Hammer Lane <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />Underground Storage Tank (UST) Inspection <br />5, 27, 36) On 3-3-11, Garrett Backus, of the Environmental Health Department, was on site to witness <br />secondary containment testing of repaired components and verify correct operation of the diesel tank <br />turbine relay. <br />The technician who did the initial secondary containment testing on 2-8-11 stated in his report that the <br />diesel turbine relay contacts are fused shut and do not shut down the diesel turbine when the diesel <br />sump sensor goes into alarm. <br />On 3-3-11, Gerardo Guzman of Walton Engineering checked the diesel turbine relay and found a plastic <br />cylinder, taped to the relay cover, holding the relay closed. The plastic cylinder disabled the positive <br />shutdown function. With the relay closed, the diesel turbine will always be on and positive shutdown will <br />not work if the diesel sump liquid sensor goes into alarm or the sensor is disconnected. Gerardo <br />removed the plastic cylinder and verified that the relay was working properly, restoring positive shutdown <br />for the diesel sump sensor. <br />The UST monitoring plan for the facility states that piping leak alarms and failure/disconnection of the <br />monitoring system will trigger automatic pump (turbine) shutdown (positive shutdown). If the facility <br />does not want the system set up for positive shutdown then an annual line tightness test will be required <br />and the monitoring plan must be updated. <br />On 3-10-11, Garrett Backus inspected the fuel tank turbine relays, designated operator reports, alarm <br />history and alarm logs at the facility. Garrett spoke with assistant manager Julie Tafoya. Julie did not <br />know why the diesel relay was altered with the plastic cylinder on 3-3-11. On 3-10-11, all of the turbine <br />relays appeared to be working properly. The facility records show that the last alarms for the diesel STP <br />sump were 2-8-11 (fuel and sensor out alarms) and 7-19-10 (fuel alarm). The secondary containment <br />testing was done on 2-8-11 and the last monitoring system certification was done on 7-19-10. Walton <br />Engineering service receipts from January and February 2011 indicate that dispensers 5 and 7 had <br />problems dispensing fuel.
