The UST tank forms on file with the San Joaquin County Environmental Health Department (EHD) <br /> have incorrect or missing information. Immediately correct the tank forms and submit a copy to the EHD <br /> by August 16, 2012. Complete a separate tank form for each tank. <br /> Notes <br /> 1. The 91 tank annular sensor sleeve was bunched up in the tank annular space. The testing <br /> technician decided to replace the sensor. A Veeder Root 794390-409 wrap-around sensor was <br /> installed. The riser height is 80 inches and the tank diameter is 120 inches. The sensor was placed at <br /> the low point at the bottom of the tank. Technician Richard Thomas works fFeFfl Tank-tek which has <br /> contractor license number 803705 (class A). A permit is NOT required for this work since the inspector <br /> witnessed the repair. <br /> 2. The 89 tank spill container failed the initial tightness test. The technician replaced the drain valve <br /> and the spill container passed the tightness retest. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
