The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EFID for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> c. Secondary containment testing on the 91 tank annular space since concrete will be <br /> broken over the tank. <br /> Please be aware that the facility cannot receive fuel in the 91 octane tank prior to <br /> the EHD inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by i:he <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from elle aianufacturer(s) o1 r;;e colliponen[(s <br /> IJcing tested, Repaired, or serviced. <br />
