PHS-EHD would like to remind you that, in order <br />to be acceptable, the line test result must include, but not <br />limited to, the following : <br />1. Name and address of the facility. <br />2. Name and address of owner or operator. <br />3. Test information. <br />4. Tank and lines information. <br />5. Test data, including leak rate. <br />6. Tightness certification. <br />7. Tester's signature and test date. <br />PHS-EHD will not be able to accept any test result without the <br />above minimum information.
