Contractor information on page 3 of the application. <br />2. Applicant's signature on page 3 of the application. <br />3. Overfill containment box. Plan and application indicate a <br />few different kinds. <br />4. Pipe distance, leak detector, automatic pump shut -down <br />mechanism and other information on page 7 of the <br />application. <br />5. The flex boots. <br />6. The dispenser sumps, dimension, piping penetration and <br />continuous monitoring of those sumps. <br />7. The overfill prevention device in waste oil tank. <br />Please submit the above information to Environmental Health <br />Division as soon as possible.
