Decontamination can bedone bytriple rinsing the pipe <br />and collecting the rinsote. Describe how the rinmetewill bmcollected and stored onsite. Triple rinsed piping ionot <br />considered hazardous waste. The rinuatemust behauled eohazardous waste. Another option iahauling the <br />pipe oohazardous waste and not rinsing. <br />2. The hauler listed "Jim Thorpe Oil Inc." is not a registered hazardous waste (HW) hauler. |naddition, the listed <br />disposal site at "580 Truck St, Placerville" is not a permitted TSDF or Treatment,&Disposal Facility. <br />3. A registered hazardous waste hauler and TSDF must be provided in the permit application since rinsate or <br />piping will behauled from the site ashazardous waste. Also provide the site EPA |[} number needed for the <br />hazardous waste manifest (to beincluded insection 6n). <br />4. Page 5needs the laboratory name after "Hereby authorize" <br />Installation of sumps and piping: <br />5. | need cut sheets for the Bravo vent box, Unisource flex connectors and the OPVVshear valves. <br />G. | need manufacturer training certificates for the Bravo sumps, Unisource flex connectors, DpVVshear valves. <br />Note that one technician working at the site must have ICC Installer training and manufacturer training onall <br />components (VmederRoot, OPVVshear valves, Unisource flex connectors, Bravo sumps, Smith Red Thread).
