Decontamination can bedone bytriple rinsing the pipe <br />and collecting the rinoete. Describe how the rinoatewi|| be collected and stored onsite. Triple rinsed piping is not <br />considered hazardous waste. The rinoabamust bahauled aohazardous waste. Another option iahauling the <br />pipe aehazardous waste and not rinsing. <br />2. The hauler listed ',Jim Thorpe Oil Inc." is not registered hazardous vveote (HW) hauler. In addition, the listed <br />disposal site at "580 Truck St, Placerville" is not a permitted TSDF or Treatment, Storage & Disposal Facility. <br />3. A registered hazardous waste hauler and TSDF must be provided in the permit application since rinsate or <br />piping will behauled from the site aahazardous waste. Also provide the site EPA |Qnumber needed for the <br />hazardous waste manifest (to beincluded insection Oo). <br />4. Page 5needs the laboratory name after "Hereby muthorize" <br />Installation of sumps and piping: <br />5. 1 need cut sheets for the Bravo vent box, Unisource flex connectors and the OPW shear valves. <br />G. | need manufacturer training certificates for the Bravo sumps, Unisource flex connectors, QPVVshear valves. <br />Note that one technician working at the site must have ICC Installer training d manufacturertnaining on all <br />components (VaederRoot, OPVVshear valves, Unisource flex connectors, Bravo sumps, Smith Red Thnaad).
