Underground Storage Tanks - Facility Form - Provide all current information, sign and <br />--------- date at the bottom. - - <br />2. Underground Storage Tanks - Tank Forms - Provide all current information, sign and <br />date the bottom of each form. <br />3. Certification of Financial Responsibility/Letter from Chief Financial Officer: Complete <br />the form provided, and make a copy for our records. This document is required to be <br />submitted to this office annually. <br />4. Underground Storage Tank Monitoring and Response Plans: Complete and submit a <br />copy. Indicate in Section IX of the Monitoring Plan the type of overfill prevention <br />present for each tank. Refer to the enclosed LG 150-1 letter. Retain a copy onsite for <br />future reference by employees. <br />5. Report #5021: Update all information for all programs at this site including the UST and <br />Hazardous Waste programs, sign and date. Bring with all other forms to our accounting <br />office to pay for transfer fees and the $95/hr UST Consulting fee, if necessary. <br />6. If you are the owner, you must submit a notification form identifying the "Designated <br />UST Operator" for each facility owned. A notification form is enclosed for your use. <br />7. Go to: http://www.waterboards.ca. og v/Mhome/ust/re ug_ latory/index.html to obtain the <br />current statutes (Underground Storage of Hazardous Substances) and UST Regulations <br />that you are required to have for your monitoring and response plan. <br />8. Also enclosed for your information is a brochure of Best Management Practices (BMPs) <br />for Retail Gasoline Outlets that you can use as part of your BMPs for your facility. <br />9. Hazardous Materials Management Plan: Contact the Office of Emergency Services, Mr. <br />Dennis Fields, at 468-3969 for information on updating this plan. <br />
