The east float & chain -diesel in the Dispenser 3/4 failed to stop the flow of product at the dispenser <br />when tested. The technician re -adjusted the chain, attempted to push dirt out of the cup of the float but <br />was unable to trigger the shear to trip. All monitoring equipment shall be maintained to activate an <br />audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct <br />immediately by having a properly licensed, trained, and certified contractor replace the failed component <br />with a functional component (LG 113-x listed, if applicable). If the failed component can't be replaced <br />immediately, there is a possibility that the diesel UST system may be red tagged to prevent fuel inputs. <br />-HMC Wayne found liquid in float, replaced and retest & passed. <br />104. Financial responsibility documents for 2014 have not been completed or submitted to CERS. <br />Current financial responsibility documents are required to be submitted annually. Complete and submit <br />a copy of the financial responsibility. <br />- Owner completed 2014 on site and kept in binder. Financial responsibility must be completed every <br />year and should be uploaded to CERS. <br />205. The January 2014 - April 214 designated operator monthly inspection report were not found on site. <br />Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />Locate and ensure that copies of the previous twelve months of designated operator monthly inspection <br />reports are maintained on site. <br />-Afforda-test office submitted copies via email and Paul had printed out copies for binder. <br />Notes: <br />- Secondary containment last tri-ennial test was last completed 08/24/11. Next due by 08/2014, in 3 <br />months.
