Describe the frequency of performing the monitoring: <br />Tank Data recorded daily, analyzed monthly by Statistical Inventory Reconciliation <br />(SIR), precision tested biannually. <br />Piping Monitored hourly (continuously) by mechanical leak detectors precision tested <br />annually, SIR analysis performed monthly. <br />B. What methods and equipment will be used for performing the monitoring: <br />Tank SIR analysis performed monthly by USTMAN Industries tank precision testing_ <br />by various State certified tank testing vendors. <br />PipingRed Jacket Model XLD or PLD Mechanical Leak Detectors SIR and line testing <br />as above. <br />C. Describe the location(s) where the monitoring will be performed (facility plot plan must <br />be attached): Data collected at tanks, recorded and kept at checkout counter, analyzed <br />offsite: Mechanical leak detectors are located in turbine sump. <br />D. List the names and titles of the people responsible for performing the monitoring and/or <br />maintaining the equipment: A14cAA44,e S,�IA" 4 P,,..re-lV,?,AJe•t Franchisee, <br />responsible for monitoring; John Willow, Director of Gasoline, and Mike Karvelot <br />Director of Environmental Affairs, responsible for maintaining the equipment. <br />E. Reporting format for monitoring: <br />Tank Monthly Fuel Reports and Monthly SIR Results. <br />Piping Monthly SIR Results. <br />F. Describe the preventive maintenance schedule for the monitoring equipment: <br />Maintenance not required. Mechanical leak detectors tested annually and replaced upon <br />failure. <br />G. Describe the training necessary for UST operation, including the monitoring equipment: <br />Franchisee is trained to operate the complete UST system y Quik Stop personnel at the <br />time the system is installed Franchisee thereafter, is responsible for training, his or her <br />store employees on the system operation. <br />DATE /Q/6 .5 <br />
