Box 1025 <br /> West Sacramento, CA 95691 <br /> Fax: (916) 373-1173 <br /> SUBJECT: Quik Stop#120,9321 Thornton Road,Stockton,CA(SR0054725) <br /> The San Joaquin County Environmental Health Department(EHD)has completed the review of <br /> the application for permit to install an OPW spill bucket in the 91 product fill sump at the above <br /> noted address. <br /> The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Garrett Backus to arrange the following inspections(a 48 hour notice is <br /> required): <br /> a. Verify the five gallon capacity and drain valve operation of the 91 product spill <br /> bucket. <br /> b. One hour hydrostatic test(or equivalent)on the 91 product spill bucket. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council(ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired, or serviced, and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br /> date is required, a written request must be submitted to this office prior to the expiration <br /> date. <br /> 6. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM, etc. See California Code of <br /> Page 1 of 2 <br />
