See attached "Continuation — Official Inspection Report" for violation(s) not listed above. <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />113. New designated operators, Kelvin Cruz, Jeramy Cummins, Stan Yoder, and Rhome Desbiens were <br />hired on January 12, 2012 and James Williams was hired on August 13, 2012 and notifications were not <br />provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 <br />days. During the inspection Kelli Kishida submitted updated copies of the Owner Statements of <br />Designated UST Operator and Understanding of and Compliance with UST Requirements. Continue to <br />ensure that notification is submitted to the EHD identifying all the new designated operators for this <br />facility within 30 days. <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />designated operator monthly inspection report and failed to check that they were responded to <br />appropriately. The missing alarms include: the December 22, 2011 Double Wall Leak Alarm for the split <br />diesel tank. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a <br />copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. <br />305. The Bravo box float and chain in the South dispenser and satellite dispenser sumps failed to stop <br />the flow of product at the dispenser when tested. All monitoring equipment shall be maintained to <br />activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. <br />The service technician adjusted the chain and verified functionality. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by December 9, 2012. <br />Remember to submit a current copy of the financial responsibility documents before November 9, 2013. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector.
