Secondary containment testing is required once every 36 months. Take all necessary precautions to ensure testing is performed in a timely manner. #4 Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Take all necessary precautions to ensure that these documents are submitted to the EHD. Corrected on site. #23(c) The designated operator failed to document all the alarms for the previous month and check that they were responded to appropriately on the 11-30-09 designated operator monthly inspection report. The missing alarms include: 11-6-09 and 11-17-09 from the MC and MC re -inspection. The designated operator failed to attach the alarm history print out for the previous month for the 11-30-09, 1-7-10, 2-12-10, 3-3-10, 4-12-10, 7-14-10, 8-4-10, 9-10-10, and 10-15-10 reports. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The designated operator failed to check and note the correct test dates for the secondary containment testing on the 2-12-10 and 7-14-10 designated operator monthly inspection reports. The secondary containment test was listed as 11-14-10 on the 2-12-10 report and 11-14-09 on the 7-14-10 report but they both should have been listed as 11-4-09. The date for the annual line test was not documented/left blank on the previous 12 months of designated operator reports. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 14 #24 The annual diesel line waass performed on 11-17-09 and a test report has not been submitted to the EHD. A copy of the test report must be submitted within 30 days of the tests. Immediately submit a copy of the test report to the EHD. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by 12-10-10 ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
