LINING TEST CERTIFICATION <br /> I have performed an internal inspection on the lining and certify that the following tests were <br /> conducted in accordance with Section 13.0,Chapter A of NLPA 631: <br /> 1) The lining was holiday tested and free of holidays. <br /> 2) The barcol hardness of the lining met the lining manufacturers barcol hardness specifications. <br /> 3) The final lining thickness had a nominal thickness of 125 <br /> mils and a minimum thickness of no less than 100 mils. <br /> Signed Date <br /> SPECIAL INSPECTOR'S SIGNATURE <br /> OWNER VERIFICATION <br /> By signing below, the tank owner or his designated agent has verified that this form has been <br /> properly completed with all required signatures and acknowledges receipt of this affidavit.
