Beckman Rd. Lodi, CA 95240 Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> On site to conduct a routine inspection. <br /> 7. The following alarms did not have maintenance records on site: <br /> L2 91 fill sump sensor out alarm of 1-1-11 and 12-19-10 and fuel alarm on 2-18-11, 12-8-10, and <br /> 11-20-10; <br /> L4 diesel fill sump fuel alarm on 2-17-11; <br /> L5 diesel AB fill sump fuel alarm on 2-25-11; <br /> L7 91 submersible turbine sump(stp) fuel alarm on 2-17-11, 2-16-11, 1-30-11, 1-2-11, 12-5-10, <br /> 11-27-10, 11-20-10; <br /> L9 diesel stp sump fuel alarm on 1-8-11; <br /> L14 diesel annular fuel alarm on 1-10-11; <br /> L16 Vent Box fuel alarm on 7-6-10; <br /> These records shall be kept on site for at least 3 years. Immediately locate and maintain the <br /> maintenance records on site and submit copies to the EHD by 7-27-11. <br /> 11. The 87 stp sump sensor was placed approximately five inches off the lowest point of the sump. It <br /> was part way up the saddle of the sump and not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> Technician adjusted position of the sensor to the lowest point of the sump. Continue to ensure that the <br /> sensors are properly located to detect a leak at the earliest opportunity. <br /> 12 & 16. The test boot on the "diesel A" product pipe was left clamped on the primary and secondary <br /> pipe. The schrader valve was left in and was not facing the bottom of the stp sump. Any potential <br /> release from the primary containment into the secondary containment in the "diesel A" piping does not <br /> flow into a collection sump. All secondary containment systems shall be sloped so that all releases will <br /> flow to a collection sump located at the low point of the underground piping. Immediately contact a <br /> properly licensed, trained, and certified contractor to ensure that any releases in the "diesel A" piping <br /> flows into the "diesel AB" piping sump. <br /> 18. The "diesel AB" spill container failed when tested. All spill containers shall have a minimum capacity <br /> of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> Immediately contact a properly licensed, trained, and certified contractor to repair or replace the spill <br /> container under permit and inspection of the EHD. <br /> Note: Technician stated that failure may be associated with the existing drop tube with mechanical <br /> flapper. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> ba `li a16�( " <br /> EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
