The following is an itemized list of underground storage tank violations that have not <br />been addressed for VAN DE POL ENT INGPACIFIC PRIDE as of October 13, <br />2017. <br />Open violations from June 27, 2017 inspection <br />Violation #104 - UST operating permit application for facility and tank information not submitted or current. <br />The UST Tank Information for all the tanks are not current in the California Environmental Reporting System <br />(CERS). <br />The riser pipe secondary containment states "none" in the Fill Pipe Piping Construction section. Please change to <br />"fiberglass" since all the tank riser pipes for receiving fuel are contained within fiberglass sumps. <br />Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br />update the required information, and submit for review by the EHD. <br />Violation #115 - Failed to inform the EHD of designated operator identification or change within 30 days. <br />Zane Nimmo conducted the January 16, 2017 designated operator inspection and report. Notification of Zane as <br />one of the site designated operators has not been provided in CERS. Any changes shall be reported to the EHD <br />within 30 days. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility in the <br />Owner Statement of Designated UST Operator Compliance location. <br />Violation #210 - Designated operator monthly inspection reports not maintained on site for at least 12 <br />months. <br />The January, February, March, April, and May 2017 designated operator monthly inspection reports were not found <br />on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br />The designated operator provided copies of the reports by email to the EHD and the operator on 6-27-17 after the <br />inspection. Violation resolved. <br />Violation #306 - Monitoring equipment is not installed or maintained to detect a leak at the earliest <br />opportunity. <br />The under dispenser containment 5/6 sump sensor was raised approximately 4-5 inches off the lowest point of the <br />sump and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be <br />able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump <br />by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest <br />opportunity. Violation resolved during the inspection. <br />Violation #309 - Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow <br />product. <br />The 87 product turbine sump liquid sensor (middle tank of 5 tanks) failed to activate an audible and visual alarm <br />when tested to ensure continuous monitoring of the tank system. All monitoring equipment shall be maintained to <br />activate an audible and visual alarm when it detects a leak. Correct immediately by having a properly licensed, <br />trained, and certified contractor replace the failed component with a functional component (LG 113-x listed, if <br />applicable). Provide notification to the EHD with the following information for like -for -like replacements: Technician <br />company name, ICC service technician certification, Veeder Root certification, brand and model number of the new <br />sensor, time and date of sensor installation. Apply for a repair permit with the EHD within one business day for <br />replacements that are not like -for -like. If the failed component can't be replaced immediately, there is a possibility <br />that the 87 octane UST system may be red tagged to prevent fuel inputs. <br />Violation #315 - Water in secondary containment not removed, analyzed, and properly disposed of (pre -Jul <br />2003). <br />Approximately two inches deep of liquid was observed in the under dispenser containment (UDC) for dispenser 5/6. <br />If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed <br />of properly. The company representative Ted Shackelford removed approximately two gallons of liquid from UDC <br />5/6. No fuel piping leaks were observed. Ensure that all sumps and annular spaces are maintained free of liquid. <br />Violation resolved during the inspection. <br />Page 1 of 1 <br />
