Cherokee Lane, Lodi Ca (Tank Closure) <br />I have reviewed the information submitted to this office in <br />response to the memo of deficiencies dated December 13, 1994. The <br />following items are still deficient: <br />1. Please complete all information required on page 3 including: <br />EPA SITE #, INSURER, FIRE DISTRICT AND PERMIT NUMBER, <br />LABORATORY NAME AND PHONE NUMBER, SAMPLING FIRM AND PHONE <br />NUMBER. <br />2. Please submit a "Site Health & Safety Plan" as required on <br />page 4, # 2. <br />3. The Owner/Operator needs to sign page 9, not the applicant. <br />4. Item # 15 on page 6 of the closure application needs to be <br />completed correctly. It appears as if the responsible party <br />to be billed for potential additional PHS-EHD time, is <br />different from the applicant.
