Cherokee Rd., Lodi <br />I have reviewed the closure plan for the above mentioned facility. <br />The following items must be addressed prior to permit approval. <br />1. Complete all applicable portions of pages 1, 9, and 10. <br />2. Please submit a " Site Health and Safety Plan". <br />3. Soil samples taken at time of removal will be analyzed for <br />leaded gasoline constituents since the date of installation is <br />"unknown". <br />4. Dry ice requirement for gasoline tanks is 30 lbs. per 1,000 <br />gallons. <br />5.
