3. Permit expires 180 days from date of approval. Note: If an extension to the <br />expiration date is required, a written request must be submitted to this office prior <br />to the expiration date. <br />4. All components must be certified by an independent testing organization and <br />labeled as required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See <br />California Code of Regulations (CCR), Title 23 Chapter 16 Table B for complete <br />list). The component must bear the appropriate mark as required by the <br />independent testing organization. <br />5. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />6. Contact Garrett Backus to arrange the following inspection (a 48 hour notice <br />is required): Functionality test for the dispenser #4 Bravo UDC pan. <br />7.
