Cherokee Lane Lodi, CA 95240 SR0066327
The San Joaquin County Environmental Health Department (EHD) has completed the review of
the permit application for voluntary ADA improvements which includes the breaking of concrete
near the UST piping system and UDC #7/8 on the underground storage tank (UST) system at
the above noted address. The permit has been approved with the following conditions:
Contact the local fire department and the San Joaquin Valley Air Pollution Control
District for any applicable permits and requirements.
2. Any changes in information and scope of work must be submitted to EHD for approval
prior to commencing work. All new addenda supersede the previous ones.
3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments
(a 48-hour notice is required for inspection appointments). Please be aware that
inspections for emergency work must be scheduled within five days of permit approval.
a. 24-hour hydrostatic test (or equivalent) on UDC #7/8 sump.
b. Secondary containment testing on UST piping connected to the UDC #7/8 sump.
4. Since concrete will be broken near the UDC #7/8 and UST piping connected to the UDC
#7/8, a secondary containment test must be performed on the UDC #7/8 and UST piping
connected to the UDC #7/8 once concrete is poured in accordance with the
manufacturer's guidelines. The EHD also recommends a secondary containment test of
these UST components before concrete is poured.
5. Qualified service technicians must possess or work under the direct and personal
supervision of an individual physically present at the work site who possesses all of the
following:
a. One of several permissible valid, current contractor's licenses issued by the
Contractors State License Board (may be issued to the installation company), or a
Tank Tester's license issued to the individual by the State Water Board, and
b. A valid, current California UST Service Technician certificate issued by the
International Code Council (ICC), and
c. A valid, current certificate of training from the manufacturer(s) of the component(s)
being tested, repaired, or serviced.
