Lodi, CA 95240 <br />DIRECTOR <br />Donna Heran, REHS <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />Linda Turkatte, REHS <br />SR0063364 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the permit application for converting the existing 89 tank to 91, converting the existing 91 to <br />diesel, install four Gilbarco Encore 500S Dispensers with Bravo Conversion Frames, saw cut <br />concrete slab, excavate to tank top, remove phase 1 vapor recovery equipment from the <br />existing 91 tank, cap 91 vapor return piping, demanifold vent risers, extend vent risers above <br />building roof, install pressure/vacuum vent caps, install diesel fill port equipment, cap former <br />vapor riser at bung, install EVR phase 2/ISD, install flex connectors, and install new concrete <br />slab on the underground storage tank (UST) system at the above noted address The permit has <br />been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />b. Secondary containment testing on diesel tank. <br />c. Verify corrosion protection on the vapor riser and that vapor riser and return pipe are <br />capped off. <br />d. Verify proper installation of the Bravo Conversion Frames. <br />e. Leak detector test: Leak detector must be tested to confirm that the device can <br />detect a leak of 3 gallons per hour at 10 psi. <br />f. Spill bucket test: Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test (or equivalent) Proper drainage must also be verified. If applicable. <br />g. Line test of the 87, 91, and diesel lines if applicable. (e.g. if new shear valves and or <br />flex connectors are installed. <br />
