Cherokee Lane Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS II, or MINOR -Notice to Comply) <br />On site to perform the routine inspection and witness the monitoring system certification testing. <br />10 & 12. The 89 annular sensor wire broke and, therefore, the tank is not monitored. The interstitial <br />space shall be monitored with a monitoring system connected to an audible and visual alarm system. <br />The sensor was replaced and retested during the inspection. The j -box was not able to be opened and <br />the conduit below the j -box broke. The sensor wire splice is outside the j -box. Technician informed me <br />that Able Maintenance will be scheduled to repair the j -box, conduit, and properly reinstall the sensor <br />splice inside the new j -box. Obtain a permit from the EHD for this work within one business day. If a <br />permit is not obtained within one business day, a "Failure to Obtain a Permit' penalty will apply. #12 is a <br />repeat violation. <br />11. The 89 stp sump sensor was located approximately halfway up the saddle of the tank sump and not <br />located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able <br />to detect a leak at the earliest possible opportunity. Technician corrected this during the inspection by <br />relocating this sensor to the lowest point of the 89 stp sump. Repeat Violation. <br />11. The 91 stp sump sensor was located on the wrong side of the tank sump saddle and not located to <br />detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a <br />leak at the earliest possible opportunity. Immediately relocate this sensor to the lowest point of the 91 <br />stp sump. Repeat Violation. <br />17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br />of detecting a 3 -gallon per hour leak at 10 psi. The service technician adjusted the 91 leak detector, <br />retested it, and it passed. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 2-20-11. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSP TIO AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Insped <br />Cece:iv�By: <br />Title: <br />M6 . <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
