Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site who possesses all of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, <br />and <br />b. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being installed, and <br />c. A valid, current UST Installation/Retrofitting certificate issued by the International <br />Code Council (ICC). <br />6. Update and submit a copy of the "UST Monitoring Plan" for EHD approval. An approved <br />copy will be returned for on site records. <br />7. A "UST Certification of Installation/Modification" form shall be submitted by the owner or <br />the owner's agent (CCR Section 2635(d)) before this upgrade is considered complete by <br />EHD. <br />8. All components must be certified by an independent testing organization and labeled as <br />required by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of <br />Regulations (CCR) Title 23, Chapter 16, Table B for a complete list. <br />9. Permit expires 180 days from the date of approval. If an extension to the expiration date <br />is required, a written request must be submitted to this office prior to the expiration date. <br />The sensor replacement was conducted during the facility's annual Monitoring System <br />Certification testing conducted on January 20, 2010.
