The second violation is for alarm documentation on the DO inspection in November. I contacted Belshire, which responded by performing the DO yesterday and to investigate the error on the DO inspection report from November. They attached a copy of their DO inspection from to me which does state that the alarms were recorded. There is no alarm response required for an Overfill alarm as the alarm will clear when the product reaches a lower level through the process of fueling. The third violation noted states that there was no evidence of training performed. Belshire attaches the latest training documents to every DO inspection performed for each facility behind the DO inspection. They also include all the training criteria, topics trained and another list behind the "DO Training" tab in the Gold binder. During Belshire's visit yesterday, they confirmed that the list is still valid and all employees have received the required training. That list is the final page of the November DO attachment. The final attachment is the Alarm Log from January and February. I will be reviewing the violation with the manager, along with our entire network and confirm that this is completed completely and accurately. I am available to confirm this matter and perform another inspection if you would like. Please let me know. Thank you for your consideration in this matter.
