Cherokee Lane <br /> Lodi, CA 95240 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to repair underground vent/vapor piping on the underground storage tank <br /> (UST) system at the above noted address. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Cesar Ruvalcaba at (209) 953-6213 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that inspections <br /> for emergency work must be scheduled within five days of permit approval. <br /> a. Secondary piping test for all impacted product piping (diesel, 87-octane, 91-octane). One <br /> hour pressure test for all piping, per manufacturer's guidelines. If there are no <br /> manufacturer's guidelines, test shall be performed using an applicable method specified <br /> in an industry code or engineering standard. <br /> b. 24-hour hydrostatic test (or equivalent) on impacted sumps or UDCs <br /> Please be aware that the facility cannot operate the UST system prior to the EHD <br /> inspection and approval. <br /> 4. If concrete was broken near tanks, a secondary containment test must be performed on the <br /> affected LISTS once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> 1868 E.
