It appears that sometime between 3/11/94 and issuance of the 1995 operating <br />permit on 8/11/95, the third UST was dropped from the permit. There are no <br />reasons for any of the tanks at this site to have been targeted for removal or <br />taken out of service. <br />2&4. Enclosed are Written Monitoring Procedures and Response Plan. Replacement <br />copies are being provided to the market. <br />3. Monitoring Log should have been available at the site. Maintenance records <br />are available at the Quik Stop Corporate Office and at Q.S.'s maintenance <br />company, Elite IV Contractors, Stockton (209) 461-6337. <br />5,6,7,9. Elite IV Contractors dispatched to service spill buckets, check ULP <br />sump, and check dates on Weights & Measures _stamps.
