Item related to cause of "Setup Data Warning" on July 28, 2010. This appears <br />to be related to testing of the 87 annular sensor on the 28d' following an L6 <br />alarm reported on July 26`h. The sensor was replaced on July 29h. The Work <br />Order (practically unreadable) and Invoice are attached. Please note that Quik <br />Stop was billed and paid for a permit. If your records show no permit, then <br />Elite IV owes you/us one. <br />2. Item related to mussing November 2009 Designated Operator Monthly Report. <br />A meeting was conducted at the Quik Stop Corporate Office on December 9�' <br />with Dulcinea and Jeffrey Covan (DO Supervisor) of Walton Engineering. <br />One of the topics discussed was the requirement to conduct the Monthly <br />Inspections each Calendar Month (not just every 30 days or so, as the <br />regulatory intent might be inferred). I was assured that the SJCEH <br />requirement would be followed. <br />3. The original Certification of Return to Compliance should still be applicable, <br />and is attached. <br />I believe all other outstanding issues related to the 8/25/10 Inspection Report have been <br />addressed. <br />
