A cold start was performed on April 24, 2017, and no notification was provided <br /> to EHD, thus requiring a permit. The sensor functionality was tested during the annual monitoring <br /> system certification inspection on 10/20/2017. No further sensor functionality testing is need at this <br /> time. <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> certificate issued by the International Code Council (ICC), and <br /> c. Possesses a valid, current certificate of training from the manufacturer(s) of the <br /> component(s) being tested, repaired, or serviced. <br /> 4. Individuals installing underground storage tank system components must possess or work <br /> under the direct and personal supervision of an individual physically present at the work site <br /> who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, and <br /> b. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br /> Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br /> installed. <br /> 1868 E.
