The designated operator failed to check all alarms were responded to appropriately on the <br /> designated operator monthly inspection report. The missing alarms not documented include: L1: <br /> Regular Sump - 10/21/10 Setup Data Warning. Alarms response action inconclusive include: L2: Plus <br /> Sump - 07/21/10 Fuel Alarm and 1-4: Diesel Sump - 08/08/10 Fuel Alarm which both L2 and L4 says <br /> "DiMaggio Services" - No work order/ records were found. <br /> Several Q1-4 PLLD shutdown alarms not documented and/or had no response action: <br /> Q1: 11/05/10, 09/15/10, 07/24/10, 05/02/10, 04/04/10, 04/03/10, 04/02/10, 03/29/10, 03/21/10; <br /> Q2: 11/05/10, 09/15/10, 08/18/10, 07/27/10, 05/02/10, 04/05/10; <br /> Q3: 11/05/10, 09/15/10, 05/02/10, 04/05/10, 03/21/10, 01/20/10, 12/22/09- <br /> Q4- 11/05/10, 09/115/10, 08/08/10, 05/02/10, 03/21/10, 01/20/10, 01/05/16, 12/22/09; <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. Submit a copy of the DiMaggio Services work order for 07/21 and 08/08/10 to <br /> EHD by 02/01/11 and a statement how to ensure all alarms with its response action will be documented. <br /> 23c. The designated operator failed to check and note the correct test dates for the secondary <br /> containment testing on the designated operator monthly inspection reports from July 2010 to present. <br /> Secondary containment was last performed 06/07/2010. During the monthly inspection, the designated <br /> operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly <br /> inspections at this facility are including all of the required information on the reports. <br /> 24, 25, & 26. Annual monitoring system certification, leak detector testing, and spill container testing <br /> were last performed on 12/22/2009 and was completed on today on 01/05/2011, one months past due. It <br /> was written on the December 22, 2010 designated operator report that "A M.C. and Spill Containment <br /> test PAST DUE". These tests are required once every 12 months. Take all necessary precautions to <br /> ensure testing is performed in a timely manner. <br /> Submit to SJC EHD within 30 days (02/07/11): <br /> 1. one copy of the "Return to Compliance Certification" form <br /> 2. a written corrective action statement <br /> 3. requested or supporting documents <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINS PECTIO AT ANY TI AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector Re cei d By: Title: <br /> EHD 23-02-003 Rev 08/10 0 CONTINUATION FORM <br />
