The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />a. Contact Michelle Henry to arrange the following inspections (a 48 hour <br />notice is required): Verification of monitoring system to ensure it was <br />unaffected by the EVR phase 11 upgrade and Incon installation. If necessary a <br />functionality test must be performed to verify monitoring system meets the <br />requirements of California Code of Regulations Title 23 and the Health & Safety <br />Code. <br />b. A copy of the alarm history prior to commencement of work and after <br />completion of work must be available for review at the time of EHD inspection. <br />2. Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site, at all times while work is being performed, <br />who possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced. <br />And in addition <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />3. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a.
