The following items need to be <br />submitted to this office before the permit can be approved, <br />CLOSURE <br />1. The application is not complete. Please complete all portions of the application. I have <br />highlighted the items that need to be completed. Please fill in all of the blanks, do not write <br />"same". <br />2.The contractor needs to complete and return the enclosed questionaire. <br />3. The contractor needs to submit a site health and safety plan. <br />INSTALLATION <br />1. The application is not complete. I have highlighted the areas that need to be completed. <br />2. You need to submit three sets of the product specification sheets for the equipment you intend <br />to install at this site (i,e monitoring panel, probes, overspill container, flex boot connectors, <br />etc.) <br />The floatation calculations need to be stamped by a registered engineer or an architect. <br />4.
