The missing alarms include 2 liquid warning alarms on 11/20/2016 and <br /> 12/12/2016 for the H4 piping vault sensor. Ensure that this facility documents all daily alarms from the <br /> Veeder Root panel and responds to each alarm appropriately. <br /> Monitoring records include: (1) date and time of all monitoring or sampling; (2)monitoring equipment <br /> calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis <br /> performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground <br /> water elevations, or other test results; (6)results of inventory readings and reconciliations. <br /> Send in information to ERD describing how the alarms were responded to. <br /> This is a minor violation. <br /> RESPONSE: 11/20/16 and 12112116 alarms were attributed to heavy rains. Frontier inspected the <br /> piping vault,found no sheen and removed the rainwater to clear the alarms. <br /> 1 <br />
