An inspection was last done on March 31, 2017 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by April 30, 2017. This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation, Class II. 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. A cold start was performed in May 2017 and testing report was not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a Class II violation. 203 CCR 2712(b) Failed to maintain records of monitoring, repair, lining, and upgrades on site. Records of repairs, linings, and upgrades for the UST system were not found on site. Site had a spill container replaced and according to DO reports a cold start was perormed in May 2017 and no recrods were found on site. These records shall be kept on site for the life of the tank. Immediately locate and maintain these records on site and submit copies to the EHD. This is a Class II violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. Monitoring plans for tank ID numbers TA0507923, TA0507924, TA0507926, TA0507927 and TA0507928. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class Il. 302 HSC 25291(b) UST system incapable of detecting leak from primary into secondary containment (pre -Jul 2003). Schrader valve on the 100 octane STP sump was open but facing up. Any potential release from the primary containment into the secondary containment in the 100 ocatane piping does not flow into a collection sump. All secondary containment systems shall be sloped so that all releases will flow to a collection sump located at the low point of the underground piping. Immediately contact a properly licensed, trained, and certified contractor to ensure that any releases in the 100 octane piping flows into the 100 ocatne STP sump. This is a Class II violation.
