E <br />The following is an itemized list of hazardous waste violations that have not been <br />addressed for QUICK SHOP as of March 31, 2017. <br />Open violations from March 22, 2016 inspection <br />Violation #101 - Managed a hazardous waste without an identification number. <br />This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, <br />transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC web <br />site http:/Iwww.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number. Immediately <br />reactivate your ID number and submit evidence to the EHD. <br />Violation #102 - Failed to determine if a waste is a hazardous waste or retain waste analysis on site for 3 <br />years. <br />According to Mr. Chahoud, the liquid that is being routinely removed from the onsite gasoline under dispenser <br />containments (UDCs)/ sumps is being disposed of into the onsite oil water separator. Any person who generates a <br />waste shall determine if the waste is a hazardous waste. Immediately stop the disposal of all liquids removed from ti <br />service station sumps, make a hazardous waste determination for the liquid waste, and manage it according the Titl, <br />22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was <br />managed. <br />Violation #106 - Failed to train employees on waste handling and emergency procedures. <br />At the time of inspection, it could not be demonstrated (not necessarily documented) that employees who handle <br />hazardous waste were properly trained. The generator must ensure that all employees who handle hazardous waste <br />are thoroughly familiar with proper waste handling and emergency procedures. Provide proof of training to the EHD <br />for employees whose responsibilities include hazardous waste. <br />Violation #107 - Failed to prepare manifests for transportation of hazardous waste. <br />According to Mr. Chahoud, waste absorbent generated during the spill clean up on 3-11-2015 was transported off <br />site by Elite IV contractors without first preparing a uniform hazardous waste manifest. An official inspection report <br />issued by EHD on 3-11-2015 indicated that the waste generated was placed into the Quick Shop hazardous waste <br />container pending disposal. A generator shall prepare a Uniform Hazardous Waste Manifest on EPA Form 8700-22 <br />prior to the transport of a hazardous waste for off-site transfer, treatment, storage or disposal. Ensure that a manifes <br />is prepared each time hazardous waste is transported for off site transfer, treatment, storage or disposal. Submit <br />proof of correction to the EHD. <br />Violation #113 - Failed to keep copies of consolidated manifesting receipts for three years. <br />Copies of hazardous waste disposal records for the last three years were not found on site. Hazardous waste <br />generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br />consolidated manifesting procedure on site for three years and have them readily available for review. Immediately <br />locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br />EHD. <br />Open violations from February 20, 2015 inspection <br />Violation #101 - Managed a hazardous waste without a valid EPA ID number. <br />This facility's EPA ID number is inactive. A hazardous waste generator shall not treat, store, dispose of, transport or <br />offer for transportation, hazardous waste without an EPA ID number. Immediately contact DTSC and reactivate your <br />EPA ID number and submit evidence to the EHD. A form has been provided. This is a repeat violation, Class II. <br />Page 1 of 1 <br />
