The monitoring plan is not current and is not approved by the EHD. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). This was corrected on site. This is a repeat violation, Class II. 121 CCR 2712(f)Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 12/5/2016 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 1/4/2017 . This information was received late. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a Class II violation. 209 CCR 2715(c)Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to attach all of the alarms to the August 2017 designated operator monthly inspection report and failed to check that they were responded to appropriately. The 1-8: 87 Annular sensor was missing. During the monthly inspection,the designated operator shall review the alarm history for the previous month,check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 215 CCR 2712(1)Failure to retain a copy of the permit to operate at the facility. A copy of the 2017 permit to operate was not on site. Immediately get a copy and retain at facility. This is a minor violation. 313 HSC 25290.1,25290.2,25291 Failure to construct, operate, and maintain primary containment as product-tight. The fuel filter in the 3/4 dispenser was observed dripping indicating a leak in the primary piping. The All primary containment for the UST system must be tight. Immediately have a properly licensed,trained, and certified contractor repair or replace the failed component under permit and inspection of the EHD. This is a repeat violation, Class II.
