4 <br /> O <br /> The following is an itemized list of underground storage tank violations that ha <br /> been addressed for ONE STOP MARKET* as of March 29, 2017. <br /> Open violations from December 05, 2016 inspection <br /> Violation #207 -Failed to record unauthorized release from primary containment. <br /> According to the reviewed DO records, an L6 fuel alarm occurred on 11-10-16 and upon inspection fuel was <br /> observed in the diesel STP sump and the release was not recorded. Mr. Kamaj indicated that there is a small leak <br /> from one of the pipes in this STP and that he is working with a contractor to make repairs. Any release from the <br /> primary containment that does not escape the secondary containment and was cleaned up within eight hours of <br /> being detected shall be recorded with the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br /> records. Submit a copy of the recordable release statement to the EHD. <br /> Violation #209 -Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> 1) The designated operator failed to document all the alarms from the attached alarm history on the 12-23-2015 <br /> designated operator monthly inspection report and failed to check that they were responded to appropriately. The <br /> missing alarms include: all alarms which occurred on 12-8-2015. During the monthly inspection, the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> 2) The designated operator failed to check and note the correct test dates for the monitoring system certification <br /> inspection which occurred on 12-8-15 on the 12-23-15, 1-22-16, 2-26-16, 3-29-16, 4-25-16, 5-18-16 designated <br /> operator monthly inspection report where it was listed as 12-18-14, and on 6-17-16, 7-22-16, 8-22-16, 9-29-16, <br /> 10-24-16, and 11-11-16 designated operator monthly inspection report where it was listed as 12-23-15. During the <br /> monthly inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and document the dates they were done. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> Violation #313 -Failure to construct, operate, and maintain primary containment as product-tight. <br /> According to the reviewed DO records, an L6 fuel alarm occurred on 11-10-16 and upon inspection fuel was <br /> observed in the diesel STP sump, indicating a leak in the primary piping. Mr. Kamaj indicated that there is a small <br /> leak from one of the pipes in this STP and that he is working with a contractor to make repairs. All primary <br /> containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br /> contractor repair or replace the failed component under permit(as appropriate) and inspection of the EHD.
