Current UST Tank Forms(former A&B forms). <br /> 2. Line, LD, Monitor Certification Test Results will be mailed directly from <br /> Triangle Environmental, Inc. <br /> 3. Business Activities Form with EPA ID Number(CAL000045920). <br /> 4. Certification of Return to Compliance <br /> Our Maintenance Contractor(Elite IV)was contacted on 5/22 and authorized under P.O. <br /> #5-180 to reposition the probes in the turbine sumps. To date, Quik Stop has not been <br /> invoiced for this work. <br /> American Valley Waste Oil Inc. was notified to remove excess drums and dispose of <br /> waste. As of this date, copies of manifests have not been received. It is my <br /> understanding that for CESQG's, accumulation dates start when 100 kg has accumulated. <br /> I believe that Elite IV has been inadvertently dating the drums when waste is first put in <br /> them rather than dating them when 100 kg has been exceeded. <br /> If you have any questions or need additional information, please call. <br />
