Louise Ave, Stockton Program: UST <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> This is a routine and monitoring certification inspection. The following are violations observed: <br /> #4,13,21,27 <br /> 4. (Repeat Violation) Financial responsibility documents have not been submitted to the EHD. Current <br /> financial responsibility documents are required to be submitted annually. Complete and submit a copy <br /> of the financial responsibility by July 8, 2011. <br /> 13. (M1) (Repeat Violation) The 91-octane Bravo box float and chain in Dispenser#6 UDC sump failed <br /> to stop the flow of product at the dispenser when tested. All monitoring equipment shall be maintained <br /> to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a <br /> leak. Observed service technician adjusted the chain and verified functionality. <br /> 21. Liquid was observed in the 87 and 91 octane fill sumps. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br /> remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> and manage it accordingly. Ensure that the sumps are maintained free of liquid. Observed service <br /> technician disposed the liquid from the sumps to the hazardous waste container on site. <br /> 27. The monitoring panel frozen after testing for failsafe; therefore the monitoring system does not meet <br /> the requirements for annual line tightness testing exemption. Annual line tightness testing is required <br /> unless the UST system is equipped with the following: <br /> 1. A continuous monitoring system that either activates an audible and visual alarm or stops the flow <br /> of product at the dispenser when it detects a leak <br /> 2. A monitoring system that shuts down the pump or stops the flow of product at the dispenser when <br /> it detects a leak <br /> 3. A monitoring system for piping other than that in UDC sumps (tank top sumps, transition vents, <br /> etc.) is failsafe and shuts down pump when a leak is detected <br /> Immediately schedule this test and provide 48 hours notification to the EHD, or reprogram/repair the <br /> monitoring panel to satisfy the annual line tightness testing requirement. Observed technician replaced <br /> the board and reprogrammed the panel to meet failsafe, performed cold start, and verified functionality <br /> of the monitoring system as an emergency repair. Submit a permit application to EHD within one <br /> business day of the repair. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by: July 8, 2011 <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD I ecto.
